DescriptionThe Office of Population Health and Accountable Care
(OPHAC) is focused on leading and facilitating the framework to
improve population health outcomes across UCLA Health's Alternative
Payment Model (APM) populations across the care continuum. As APMs
evaluate outcomes regardless of where the care was rendered, OPHAC
initiatives are targeted at improving population-based outcomes for
care at UCLA Health and outside of UCLA Health (e.g. post-acute
care settings, pharmacies). Successful participation in APMs is
essential to respond to the evolving healthcare industry, as health
systems are experiencing increased pressures to take on more
financial risk and be held accountable for value-based
outcomes.
The Assistant Director of Clinical Quality Performance will
collaborate with Office of Population Health and Accountable Care
(OPHAC) leadership to develop, execute, and oversee Population
Health's Clinical Quality Performance function. The role will be
responsible for program development, implementation, and oversight
of various clinical programs and medical services in Population
Health including but not limited to: Health Risk Assessment
Program, Care at Home Program, Population Health Pharmacy Programs,
and Population Health Quality Management Program. The Assistant
Director will lead and oversee a multidisciplinary team of clinical
supervisors, nurse practitioners, pharmacists, pharmacy
technicians, quality outreach specialists, and program
coordinators. This role will support the Faculty Practice Group
(FPG) across accountable care, population health, and
department-level initiatives and will interface directly with
leadership, clinical departments, and organizational stakeholders
to ensure that the priority initiatives are supported and meet the
needs established quality and utilization improvement accountable
care goals.Salary Range:
$112,900.00-$256,900.00/annuallyQualificationsRequired:
management and strong organizational, communication, and leadership
skills

knowledge of clinical program planning, cash and accrual
accounting, financial
management control systems, financial analysis and management
reporting, and
the operation aspects of clinical programs in ambulatory teaching
settings
to be a team player, flexible and capable of working in an
environment which is
fast paced, progressive and requires entrepreneurial thinking and
action
