Job Description:
A company is looking for a Clinical Care Specialist.
Key Responsibilities
Manage and monitor a patient census, coordinating Chronic Care
Management services for chronically ill patients
Create individualized Care Plans and conduct care coordination with
patients and their care teams
Provide patient education and assess clinical and non-clinical gaps
in patient care
Required Qualifications
Registered Nurse (RN) license in applicable state
Five years of nursing experience required
Experience with chronically ill patients preferred
Telephonic case management or care coordination experience
preferred
Strong computer skills and reliable internet connection
required
