A company is looking for an Inpatient Care Manager - MSW.
Key Responsibilities
Conducts and documents psycho-social assessments to evaluate
patient and family needs
Manages discharge planning to optimize length of stay and address
barriers to care
Provides counseling and advocacy to assist patients and families in
coping with health-related challenges
Required Qualifications
Master's Degree in Social Work
Less than 1 year of experience in an MSW internship program
Ability to multi-task and prioritize work effectively
Proficient in management processes and standards in healthcare
Strong analytic skills for measuring healthcare processes and
outcomes
