This role oversees a team of Case Managers and Care Management Coordinators, while collaborating closely with physicians, social workers, and interdisciplinary teams to ensure optimal patient care and resource utilization.The Care Management team operates in a fast-paced, supportive environment focused on patient advocacy and care coordination. The Manager Patient Care Coordination will work with skilled RNs, Social Workers, and support staff to ensure patients receive the right care at the right time. The unit's energy is driven by a shared mission to improve health outcomes and reduce barriers to care.The Manager Patient Care Coordination will lead clinical practice by mentoring staff, developing care plans, and ensuring compliance with quality standards. On the administrative side, this role will manage daily operations, oversee staffing, and optimize workflows to enhance patient care delivery.
