Job Description:
A company is looking for a Bilingual Pediatric Care Manager to
plan and manage behavioral and physical care for pediatric members
and their families.
Key Responsibilities
Plan and manage pediatric medical and behavioral care across the
continuum of care
Develop member-centric care plans in collaboration with families
and healthcare teams
Document all encounters and update care plans based on member needs
and goals
Required Qualifications
NYS RN or LCSW, LMSW required
1 year of pediatric clinical field experience, including work with
child welfare and medically fragile populations
Proficient in navigating electronic documentation systems
Ability to collaborate with multiple stakeholders for care
coordination
Strong interpersonal skills, particularly in crisis situations
involving children and families
